LYLP Staff Acceptance Terms
- If hired, I will follow LYLP rules and instructions and model LYLP's moto.
- If hired, I will be kind to LYLP staff, colleagues and campers.
- If hired, I will bring a positive attitude and my best effort to all activities.
- If hired, I will not use cellphone during camp activities except to communicate with other LYLP staff.
Lifeguards, Counselors and Jr. Counselors only.
- If hired, I will attend 2-day training and all 4 weeks of camp except when approved by Program Director.
- If hired, I will bring LYLP shirt, sneakers, swim suit and towel daily.
Permissions and Terms
- I will complete a CORI form when requested and give permission to LYLP to conduct a CORI inquiry.
- I give permission to LYLP to contact references and to publish photos and videos of me as part of LYLP activities.
I understand that providing false or misleading information or failure to follow acceptance terms may result in my dismissal.
Applicant Liability Release and Consent
In consideration of the acceptance of my application for LYLP summer program, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages or which may hereafter occur to me as a result of my participation in said summer program. This release is intended to discharge in advance the Lowell Youth Leadership Program, its officials, officers, employees, volunteers, and agents from liability. It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release, and assumption of risk is to be binding on my heirs and assignees.
